Administration Process for the Paper Version

Step 1: Client/institution contact places an electronic order with the CSEQ Assessment Program via the CSEQ Web site.
Step 2: CSEQ staff fills the order, sending paper surveys to the client/institution contact within 3 weeks for standard delivery.
Step 3: Client/institution contact administers the survey locally on their campus. See Administration Guidelinessection for detailed instructions on how to administer surveys locally in accordance with approved IRB protocols.
Step 4: Client/institution contact collects completed forms from students, and ships them back to CPR with a completed Processing Form.
Step 5: CSEQ staff scan/process completed surveys.
Step 6: CSEQ staff prepares an Institutional Report of survey results, which includes frequencies and means arrayed by gender and any special analyses requested. Institutional Reports are sent to the client within 4 to 6 weeks for standard delivery. Special analyses may require additional time for delivery.
