Adherence to the content outlined below is expected of all institutions regardless of the survey distribution method or consent form template selected for use in their administration.

All Recruitment Messages Must Be Reviewed by IUB HSC

All messages sent to students asking them to participate in CSEQ or CSXQ must have approval from the Indiana University Bloomington Human Subjects Committee (IUB HSC) including messages sent directly to students by participating institutions. The CSEQ Assessment Program provides a variety of IUB HSC pre-approved message templates from which schools can choose to facilitate this process. Any changes to the pre-approved templates other than the standard institution-specific merge fields (institution name & nickname, signatory name & title) are subject to IUB HSC review.

Customizations should be limited to the introductory paragraphs of messages. The CSEQ Assessment Program encourages institutions to customize their messages in ways that will increase response rates, such as providing information about specific implications of the survey for the individual campus, or providing details of any participation incentive programs the institution is offering. Message sections describing participant rights and contact information should not be modified unless required by a local institutional IRB board. Such changes are allowed; however, they must also be reviewed and approved by the IUB HSC.

Maximum of Five Recruitment Messages

A maximum of five messages can be sent to each sampled student, including those sent by the CSEQ Assessment Program and individual institutions. In addition to the pre-approved invitation and reminder templates provided by the CSEQ Assessment Program, we also provide a template for institutions that wish to send a pre-survey announcement message to their student sample. This template may be customized, but the final text must be reviewed by IUB HSC before it can be sent. Institutions that send an announcement will have to send one less reminder message to their students.

Paper:

Adherence to the content outlined in the Required Survey Administration Protocol Instructions document is expected of all institutions regardless of the survey distribution method or consent form template selected for use in their administration. This document will be provided to institutions upon registering for a CSEQ or CSXQ paper administration.

Survey Distribution in Classroom or other Established Group Settings
Institutions distributing paper surveys in a classroom or other established group setting are provided with an IUB HSC approved message template and instructions as to how survey administrators must introduce and distribute the survey to students.

Survey Distribution through Campus or Standard U.S. Mail
Institutions wishing to send the paper survey to students via campus or standard mail can use one of three informed consent form templates as their invitation to participate. Each institution is provided with instructions on how to use the chosen message template in compliance with IUB HSC protocols. Each institution can then send up to four reminder messages to students.
